Abstract

Pendahuluan: Obstructive sleep apnea (OSA) adalah episode abnormal pada frekuensi napas yang berhubungan dengan penyempitan saluran napas atas pada keadaan tidur. Beberapa penelitian  menyebutkan bahwa obstructive sleep apnea (OSA) merupakan faktor risiko penting untuk terjadinya stroke iskemik melebihi 60%. Tujuan: Memperoleh hubungan resiko terjadinya obstructive sleep apnea (OSA) dengan kejadian stroke iskemik di RSUP Prof RD Kandou Manado. Objek: Sebanyak 55 subjek penelitian yang memiliki riwayat stroke iskemik dan sebanyak 50 sampel kontrol diambil secara acak dari pasien yang bukan stroke di Poli Saraf RSUP Prof RD Kandou Manado. Metode: Metode penelitian ini dengan studi potong lintang terdiri dari 105 subjek dengan rentang usia 50-80 tahun. Sampel yang memenuhi kriteria inklusi dan ekslusi diberikan kuisioner Berlin untuk menilai derajat OSA kemudian dianalisis menggunakan metode chi - square. Hasil: Diperoleh 105 subjek penelitian dengan rerata usia 59,45±8,57 tahun dan proporsi laki-laki dan perempuan yang hampir sama. Sebanyak 42 (76%) subjek dengan resiko tinggi OSA mengalami stroke iskemik dan 13 (23%) subjek resiko rendah OSA mengalami stroke iskemik. Terdapat hubungan yang bermakna resiko tinggi OSA dengan kejadian stroke iskemik dengan p<0,05, OR= 7,54, 95% CI ( 3,167-17,951). Kesimpulan: Subjek dengan resiko tinggi OSA memiliki peningkatan angka kejadian resiko terjadinya stroke iskemik.

ABSTRACT

Introduction: The obstructive sleep apnea is a treatable form of disordered breathing in which the upper airway closes repeatedly during sleep. Several studies have shown a prevalence of the syndrome among patients with stroke that exceeds 60 percent. We conduct this study to determine the relationship between obstructive sleep apnea and ischemic stroke at Prof R D Kandow Hospital Manado.  Object: There were 55 subjects who had  ischemic stroke and 50 subjects as a control subject. We collect the data at Neurovascular Clinic of Prof.R.D.Kandou Hospital, Manado. We conduct a cross sectional study among 105 subjects aged 50-80 using quisionaire berlin to assess the obstructive sleep apnea. We analyze the data using chi-square method. Result: There were 105 subjects recruited with mean age 59,4±8,57 years old and number of male and female were equal. Forty-two (76%) subjects were at high risk of  OSA  who had ischemic stroke and 13 (23%) subjects were low risk of OSA who had ischemic stroke.The data was analyzed by chi-square, there was a significant association between OSA and ischemic stroke p<0,05, OR= 7,538, 95% CI ( 3,166-17,951).  Discussion: Research subjects with high risk of OSA have higher risk of ischemic stroke incidence.
